Good health and quality of life is what motivates us to provide healthcare solutions and support for those who need it most.<\/p>\n<p>We are looking for an energetic and dynamic\u00a0<strong>Outreach Team Leader: Professional Nurse<\/strong>\u00a0to lead the Home-Based Carers, CDU, CDL, Rehab\u00a0Adherence, CDU Staff.<em><\/p>\n<p><\/em><strong>Key Duties and Responsibilities:<br \/>\n<\/strong><\/p>\n<ul>\n<li>Design weekly work plans for the CHW and for him or herself (home visits, household assessments,<\/li>\n<li>community outreach events, etc.)<\/li>\n<li>Manage the Community Healthcare Workers per node.<\/li>\n<li>Work closely with the DoH facility management and the community leaders to facilitate the Community-Oriented Primary Care approach \u2013 meaning the OTL is required to attend the local community meetings.<\/li>\n<li>Attend and actively participate the multi-disciplinary meetings organised by the DoH facility management.<\/li>\n<li>Actively participate in the wellness and prevention interventions, e.g., administering of vaccines, screening for non-communicable diseases, family planning, etc.<\/li>\n<li>Plan and implement mentorship and coaching sessions for the CHW on the Integrated Health and Wellness Service package.<\/li>\n<li>Routinely record\/update patient files and stats.<\/li>\n<\/ul>\n<ul>\n<li>Accurate reporting (verbal and written) of stats and narrative reports.<\/li>\n<li>Maintain patient confidentiality.<\/li>\n<li>Ensure that patient files are filled and stored correctly and safely \u2013 POPIA compliant.<\/li>\n<li>CHW timesheet and leave management and timeous submissions to Anova\u2019s HR department.<\/li>\n<li>Manage the office expenses, advance claims, and mileage claims with timeous submissions to the Anova\u2019s finance department.<\/li>\n<li>Prioritise, organise and manage own workload in a manner that maintains and promotes quality.<\/li>\n<li>\u00a0Any other tasks as agreed with line manager.<\/li>\n<\/ul>\n<p><strong>Minimum<\/strong>\u00a0<strong>Qualifications,<\/strong>\u00a0<strong>Experience<\/strong><strong>\u00a0and Skills Required<\/strong><\/p>\n<ul>\n<li>Nursing qualification (Professional Nurse)<\/li>\n<li>SANC registration<\/li>\n<li>Valid driver\u2019s license essential<\/li>\n<li>Own transport and vehicle<\/li>\n<li>Experience working within the community will be advantageous<\/li>\n<\/ul>\n<p><a href=\"https:\/\/anovahealth.simplify.hr\/Vacancy\/89558\">Apply online\u00a0<\/a><\/p>\n<ul>\n<li>These positions will be based in Parow and Strandfontein so please take this into consideration when applying.<\/li>\n<\/ul>\n<\/div>\n<p><strong>Closing date:5 June 2024<\/strong><\/p>\n<ul>\n<li>In accordance with our Employment Equity goals and plan, preference will be given to suitable applicants from designated groups as defined in the Employment Equity Act 55 of 1998 and subsequent amendments thereto.<\/li>\n<li>Anova Health Institute is a provider of essential health services and therefore has a mandatory vaccination policy.
